About this event

Description

Each year, SourcePoint partners with the Ohio Department of Insurance (ODI), and the Ohio Senior Health Insurance Information Program (OSHIIP) to provide this FREE service to Delaware County residents. During the Medicare Annual Open Enrollment Period, you will be able to meet with an OSHIIP certified counselor to compare, change, or enroll in the Medicare Part D (Prescription Drug) or Medicare Part C (Medicare Advantage) plan that best meets your health insurance needs.
Please note each appointment is only for one person. If there is a second person in your household who needs their Medicare plan reviewed as well, they MUST register for an individual appointment. If you would like to work with the same counselor as another individual, schedule consecutive appointments.
Does your previous or current employer provide Connector services?Connectors are third-party companies that provide enrollment services for retirees of some companies. If your company is helping to pay for your Medicare insurance, there is a chance you have a Connector service. Please check with your HR department to confirm. If your company offers Connector services and you don’t use this service, you could forfeit your employer contributions. Therefore, we cannot meet with individuals offered this service.
We will cancel your appointment if we have not received the requested information at least 2 weeks prior to your scheduled appointment. You are welcome to mail it back in the return envelope provided or drop it off at our Office Front desk.
Thank you for your interest in an Open Enrollment Check-Up appointment with SourcePoint. After your registration is complete, you will receive a pre-appointment packet in the mail with paperwork you will need to complete and return to us two weeks before your scheduled appointment to allow us enough time to prepare. If we do not receive your form within two weeks prior to your appointment your appointment will be cancelled. We will keep your information secure prior to your appointment.
This program is only open to current Delaware County residents.
