GENERAL INFORMATION • 8th grade through current college students • Clinic 12/6 registration starts 9:00AM-9:45AM, clinic runs from 9:45:30AM-3PM • Location: Sports Center 148 Mall Rd Lexington Ky 40503 • Online registration: $150 • $55 for solo (only 50 spots available) • REQUIRED DOCUMENTS FOR ALL UK ATHLETICS CAMPS: – Participants must upload the following documents to your Active Network Camp Account. The camper will not be able to participate in camp until we have received these forms. For all camps, disabilities will be accommodated with advanced notifications. -A copy of a physical that was signed by a physician within the last 12 months -Front and back copies of your medical insurance card(s) • WHAT TO BRING: Each camper should wear athletic clothing, dance shoes, and their own water bottle. No jewelry, No gum. • COMMUNICATIONS: Please double-check that your email address is correct in your Active Network Camp Account as that will be our only method of communication with campers. After you complete your registration, you will receive a confirmation email. A few days before the camp you will receive another email with more details and instructions about the camp. • Merchandise sales will be in person with a check made out to UKAA or cash only. We do not accept credit cards. • REFUND POLICY: Refunds can be requested up to 7 days prior to the start of camp by emailing janet.brock@uky.edu.
